Individual Living Skills Specialist Job Description (Flexible/Part-time Independent Contractor)

Description:

Healing & Restoration Consulting LLC (HRC) is seeking a dedicated, passionate ,energetic, Independent
Living Skills Specialist to join our team. The ILS Worker provides clients with the training and resources
needed to develop the skills necessary for independent living. The goal is to maximize client
independence in the community. ILS workers will drive to each client’s home or approved billable
setting, and render services.

Main Duties/Responsibilities:

  • Communicate monthly or more often as needed with client’s assigned case managers;
  • Follow case chronology procedure
  • Report to supervisor all important client issues; follow case chronology procedure
  • Recognize the need for IPP goal changes and communicate this need to ILS Supervisor
  • Initial development of Individual Program Plan (at SOC)
  • Use assigned software programs to track work and hours
  • Periodic Individual Program Plan Reviews (bi-annually or more often as needed)
  • Completion of the weekly client time cards and weekly visit time slips
  • Be prepared for monthly supervisory meetings and periodic Individual Program Plan reviews
    based on DHS standards
  • Communicate appropriately with the ILS Supervisor
  • Follow through on requests from the case managers or supervisors within 24 business hours
  • Maintain an ILS folder in the clients’ home
  • Basic knowledge of diagnosis of assigned clients
  • Maintain professional boundaries with all client’s assigned and establish a therapeutic
    relationship
  • Ability to work in a variety of billable approved settings
  • Drive clients following established guidelines

Qualifications:

  • High School Diploma and 2 years experience working with individuals with disabilities
    or
  • Associate Science degree in Human Services or other related field
  • Driver license required and valid insurance per State of MN standards
  • Registered Vehicle
  • Pass Minnesota DHS Background study
  • Must has full coverage auto insurance

Rates of Pay:

$17-$19 per hour DOE ($.47 per mile for mileage reimbursement)
Fee-for-services based on face to face and telehealth contact with clients;
Full time (40 hrs)
Part Time (20-32 hrs)
Position is part-time with potential for full-time

Office Assistant (Part-time/ flexible opportunity to grow to full-time)

this role, you’ll play a critical role in keeping our office a positive, welcoming and productive
environment. You’ll also get the chance to interact with all levels of staff and various customers and

Role Description:

  • Make sure office is organized and stocked with office supplies
  • Assist Operational Director with employee orientation making sure; employee documents are
    filed, emails are set up, update directory, and phone/ fax number extension added or other task
    assigned by Operational director related staff orientation
  • Must be present in the office 3x a week open mail scan to Operational Director and Clinical
    Director.
  • Complete mailings as assigned by Directors
  • Update policy manuals as assigned by Directors
  • Check fax and email to Operational and Clinical Director
  • Update and send, training, supervision, and all staff meeting dates to staff/ as calendar invites
  • Assist directors with scheduling and setting up meetings ordering food and making purchases as
    approved by Operations Director
  • Create forms as needed
  • Office filing as needed
  • Coordinate with the Clinical Director to send records as requested.

Rates of Pay:

$16-19 per hour

Mental Health Practitioner

Description:

(HRC) Healing & Restoration Consulting LLC strives to meet the mental health needs of the
under-served communities with emphasis in serving individuals and families within their environmental
context in order to receive a full view of individual/ family function, social supports, and strengths. HRC,
is a diverse, innovative, strength-based team of professionals and practitioners who strive to empower
and support individuals and families in their goals of becoming self-sufficient. By teaming together in a
person-centered manner to provide culturally appropriate assessment, therapy, and mental health
rehabilitative services to further enhance individuals/ communities served quality of life. Candidates

Responsibilities:

  • Provide ARMHS and/or CTSS Skills Training meeting all DHS and Medicaid requirements
  • Teach skills to improve participants mental health
  • Develop a recovery goal, rehab goal, and objective with client
  • Ensure confidentiality of all records and interactions
  • Coordinate services with county case managers, family/guardian of client, work personnel, medical personnel, etc.
  • Participate in professional meetings and workshops.
  • Develop therapeutic relationship with team and client.
  • Provide clinical interventions under the supervision of a mental health professional.
  • Attend supervision meetings and participate in team-based decisions regarding all clients
    and services • Documentation_ maintain client files, update client files, update treatment
    plans a minimum of once every six months, and do all required documentation as specified in
    DHS guidelines
  • Complete accurate case notes in session for each billable unit using the Breezynotes Electronic
    Medical Records system
  • Must have current driver’s license in good standing, comprehensive auto insurance, and
    maintain own reliable transportation
  • Maintain staff and professional relationships with clients
  • Public Relations/Networking/Liaison o Responsible for the reputation and public
    image of the agency Practitioner will educate self on community resources

Documentation

  • Maintain client files, update client files, update treatment plans a minimum of once every six months, and do all required documentation as specified in DHS guidelines.
  • Complete accurate case notes in session for each billable unit using the Breezy system.

Minimum Qualifications:

Mental health practitioners providing services for the treatment of mental illness, who are not eligible
for enrollment, must be under clinical supervision of a mental health professional and must be qualified
in at least one of the following five ways:
1. Holds a bachelor’s degree in a behavioral science or a related field, from an accredited college or
university and meets either a or b: a) Has at least 2,000 hours of supervised experience in the delivery
of mental health services to recipients with mental illness b. Is fluent in a non-English language of a
cultural group to which at least 50% of the practitioners recipients belong, completes 40 hours of
training in the delivery of services to recipients with mental illness, and receives clinical supervision from
a mental health professional at least once a week until the requirements of 2,000 hours of supervised
experience are met
2. Has at least 4,000 hours of supervised experience in the delivery of mental health services to
recipients with mental illness. Hours worked as a mental health behavioral aide I or II under Children’s
Therapeutic Services and Supports (CTSS) may be included in the 4,000 hours of experience for child
recipients.
3. In addition to the above criteria: A mental health practitioner for an adult recipient must have
training working with adults. All candidates must pass a pre-employment background screening.

Rates of Pay (based on client contact)

Skills: $20 – $24 per hour ($.47 per mile to client home or community location and return only)

Licensed Therapist and Clinical Trainee (Part/Full-time Independent Contractor)

Description:

We are looking for a compassionate licensed therapist or (if eligible to practice therapy under
supervision) mental health practitioner to be responsible for counseling individuals, groups, or families
to diagnose and treat mental health disorders. The mental health therapists’ responsibilities also include
referring clients to specialists, monitoring progress during treatment, and creating a comfortable
environment for clients to be treated in. This position will also include teaching skills to the families
and children in the case load. To be successful as a mental health therapist you must be professional
and keep all client information confidential. We are looking for therapists that have experience of
working cross-culturally. We have openings in our school-based, child/adult outpatient program, and
CTSS program.

Main Duties/Responsibilities:

  • Diagnose and treat mental health disorders
  • Facilitate sessions with individuals, groups, or families
  • Refer clients to specialists if necessary
  • Create a treatment plan based on each client’s needs
  • Monitor client progress during treatment
  • Other duties as assigned by the Clinical Director
  • Practice within the APA/ SW Code of Ethics or applicable ethics code
  • Work within HRC’s clinical policies and procedures
  • Maintain organized client files
  • Maintain Professional Liability Insurance
  • Testing and assessment or internal referral to appropriate HRC staff
  • Efficiently maintain and respond to emails, phone calls, and related communication
    options

Qualifications:

    • Doctorate in Psychology (PhD or PsyD,) or Master’s Degree (LICSW,
      LGSW, LPCC, LMFT, LP) or in related field
    • Licensed and in good standing in the state of MN or (if eligible to practice
      under supervision)/ Pd student internship opportunities
    • Bilingual Staff and staff of African/ American, Hispanic, Asian American,
      Native American are preferred but not required
    • Ability to treat patients in-person or through telemedicine

The following Credentials must be surrendered upon offer extension:
1. Driver’s license and proof of auto insurance
2. Proof of licensure for mental health professionals and practitioners.
3. Personal Liability insurance- purchased via Hiscox

Rates of Pay: Travel rates only apply for {CTSS Home and Community-based services)
